Cleveland & Vermillion Districts, Apprenticeship Program Application

Applicants must be 17 years old.

There is a $10.00 fee for this application. The fee must be submitted, in cash or money order (personal checks are not accepted), to the training center, 12525 Corporate Drive, Parma, Ohio 44130. Checks made payable to “SMW #33 Cleveland District JATC.”

This application will not be deemed as completed until the training center has received the application fee, and a copy of the applicants state issued identification high school diploma or GED.

Applicants must also contact the JATC to schedule their exam and pay the $40 (cash) exam fee.

I agree that notices relating to this application may be sent to me utilizing the e-mail address I have provided.

Copies of your High School Diploma or GED, Drivers License or State ID and a copy of your Military DD-214(if applicable) must be submitted to the JATC with your application.

Your application is NOT valid until a ten dollar ($10.00) Cash Application Fee is received in person at the Sheet Metal Workers' Training Center located at 12525 Corporate Drive, Parma, Ohio 44130.
